All Alteration Applications are now being processed through the Greenview Properties office.

Please submit the following items via email in one complete submission to pjones@greenviewproperties.com.

Completed Alteration Application (download file)
Copy of Scope of Work (Estimate from Vendor)
Copy of Suffolk County Contractors License
Signed Hold Harmless by your Contractor (download file)
Proof of Workman’s Comp
Copy of Vendor Proof of Liability Insurance w/ the 3rd Party interest box "x" and listed as (view Sample)*

*This insurance form is obtained from your contractor’s insurance agent. Insurance Accord submitted w/out the “x” marked or have been altered will be rejected as invalid.

Once all documents have been verified, they will be submitted to the Board for approval.

Water Shut Down Request requires a 48-hr. notice and you will only be allotted a 4-hour time frame between the hours of 9am and 4pm.
